چکیده

mediated syncope (NMS) is not well known. Both peripheral (Bezold-Jarish reflex) and central nervous system (CNS) activation have been proposed to explain the paradoxical activation of the inhibitory cardiac vagal afferent neuronal activity and withdrawal of the sympathetic activity that occurs during NMS. Among the latter, central serotonergic mechanisms have been received little attention. By the early 1990’s a lot of research work was published on the participation of serotonin (5-hydroxytryptamine (5HT)) of the CNS in the cardiovascular homeostasis and provocation of the NMS. Application of serotonin directly

Sympathetic and baroreceptor reflex function in neurally mediated syncope evoked by tilt.

The pathophysiology of neurally mediated syncope is poorly understood. It has been widely assumed that excessive sympathetic activation in a setting of left ventricular hypovolemia stimulates ventricular afferents that trigger hypotension and bradycardia. We tested this hypothesis by determining if excessive sympathetic activation precedes development of neurally mediated syncope, and if this c...

Central serotonergic responsiveness in neurocardiogenic syncope: a clomipramine test challenge.

BACKGROUND Central serotonergic mechanisms appear to participate in the pathogenesis of recurrent neurally mediated syncope. The aim of the study was to investigate the responsiveness of the central serotonergic system by measuring the prolactin and cortisol responses to intravenous administration of the serotonin reuptake inhibitor clomipramine. METHODS AND RESULTS Twenty subjects free of an...

Neurally mediated cardiac syncope: autonomic modulation after normal saline infusion.

OBJECTIVES This study assessed the heart variability response to orthostatic stress during tilt table testing before and after normal saline administration. BACKGROUND The efficacy of sodium chloride and mineralocortoid in the treatment of neurally mediated cardiac syncope is attributed to intravascular volume expansion; however, their modulation of autonomic nervous system activity has not b...

The effects of exercise training on arterial baroreflex sensitivity in neurally mediated syncope patients.

AIMS The clinical effects of different modalities of treatment for neurally mediated syncope have been studied for years; however, their influences on its pathophysiological mechanisms still have not been determined. This research aimed to observe the effects of physical training, tilt training, and pharmacological therapy on the arterial baroreflex sensitivity and muscle sympathetic nerve acti...

Triggering mechanism for neurally mediated syncope induced by head-up tilt test: role of catecholamines and response to propranolol.

OBJECTIVES We studied the triggering mechanism for neurally mediated syncope. BACKGROUND Although increased transient sympathetic tone is thought to be necessary for the development of neurally mediated syncope, little is known about the triggering mechanism for neurally mediated syncope. METHODS Plasma epinephrine (EP) and norepinephrine (NE) levels were assessed in 20 syncope patients dur...
